**Step 1:** Click on new, a new line will be added.

**Step 2 & 3:** Click on the arrow to display drop down list to select Proposed Action and Proposed Action done By.

**Step 4:** Click on the lookup to insert Remarks.

**Step 5:** Click on Save.

**Step 6:** Click on the close button to return to Main Page.
